• Once you fill out and submit the form below your kit will be in the mail within 5 business days (usually sooner).

• Directions on how to use the kit is included in your package, but is also available here for your review.

• Once you've taken the test, please fill out this anonymous and HIPAA compliant form with your results. This is completely anonymous and is only collected to help fight to keep programs like this up-and-running. No identifying information will be collected.
